This symposium aims to provide an overview of the efficacy & safety of the anticoagulants used for stroke prevention in patients with Atrial Fibrillation while also reviewing the bleeding profile of these medications. It will also give participants the opportunity to discuss potential drug interactions related to anticoagulants while also reviewing optimal treatment options for patients with renal impairment. The symposium will also review risk stratification and risk assessment tools. It will also expose participants to the recent clinical trials of various antithrombotic treatment options in Atrial Fibrillation. This program will be comprehensive and explore broad and novel concepts relating to the large cross section of patients with acute coronary syndromes (ACS). Specifically, it will evaluate the changing epidemiology of ACS, the impact of novel advances in molecular medicine and the extent to which these are clinically applicable. Insight into the key role of a systems-approach to the management of acute coronary disease will be emphasized. The implications of diabetes, sex differences, and the increasingly important consequences of bleeding in ACS will be discussed. The changing role of electrical devices as it applies to the spectrum of patients with acute coronary disease will be undertaken. Finally, the key role of a general internist in secondary prevention of acute cardiovascular disease will be provided. These topics will be brought to life with focused case presentations designed to enrich the audience's understanding of these clinically relevant topics.

In 2011, an important new accredited symposium will be offered at CCC with a special focus on the clinical challenges faced by community cardiologists. For the first time, Canadian community cardiologists will take the stage with colleagues from other countries to share their experiences around an important community-based clinical challenge. For this first Global Round Table, the focus will be on treating symptomatic CAD. The most common presenting symptom for CAD is chest pain associated with angina. Heart rate reduction plays a central role in the management of patients with angina pectoris. Treating angina remains an important challenge for community cardiologists not only in Canada but around the world. The Global Round Table will allow for a group of Canadian community-based cardiologists to exchange with international colleagues on the best treatment strategies for treating patients with Symptomatic CAD. This accredited symposium will also provide Canadian community cardiologists with important insights on new treatment strategies for heart-rate lowering based on the direct clinical experience of their international colleagues.

A new study published in the Canadian Journal of Cardiology highlights the gaps in knowledge, skill and competencies of Canadian physicians caring for patients with atrial fibrillation (AF). Key findings of the study include: physicians reported being challenged by the need to individualize and constantly adjust each treatment, lacking the knowledge, skills and confidence to support their clinical judgments; that the use of rate or rhythm control in the management of AF remained unclear; lack of knowledge and confidence about new drugs and suboptimal knowledge of the CCS guidelines for management of patients with AF along with lack of confidence in their application. This symposium will employ a case-based approach to address these educational needs and provide a systematic and consistent approach to assessing and managing patients with AF. Experts will incorporate and explain recent changes to the 2010 CCS guidelines for the management of AF to help physicians and other healthcare professionals apply this knowledge to improve the care of their patients with AF.

ACS is a major cause of morbidity and mortality in diabetes, a disease associated with accelerated atherosclerosis. Antiplatelet therapy is a key component of ACS management, along with PCI in carefully selected patients. However, clinicians must balance potency of antiplatelet therapy with the potential for increased bleeding or recurrent thromboembolic events, a decision complicated by variability in patient response and in platelet function, as well as diabetes-related complications and their frequently occurring coexisting disorders. This Interactive ShowCASE™ incorporates three distinct educational formats, each enhanced by a hand-held digital device, to provide current evidence-based best practices in this dynamic field.

CORONARY INTERVENTION WITHOUT ON-SITE SURGERY: THE CANADIAN MODEL

Delivery of advanced cardiac procedures such as coronary angioplasty in a timely and expeditious manner is essential to the health of Canadians. There are however large rural and urban populations who do not have ready access to potentially lifesaving procedures due to the vast geography of the Canadian landscape. While coronary intervention has traditionally been viewed as a tertiary level service carried out in urban centres with on-site surgical facilities, there has been a growing number of centres across Canada establishing coronary angioplasty programs without on-site surgical back up to fill this need. These programs are at various stages of maturity from some in their infancy to those active for over a quarter century providing a paradigm for this model of service delivery. This symposium will be presented by the network of Canadian centres carrying out cardiac intervention without on-site cardiac surgery to highlight a facet of contemporary Canadian standard of practice unfamiliar to many practitioners. Topics will encompass organization of the programs and the type of local, regional and provincial issues encountered. Centres will detail their clinical experiences, results and outcomes. Specific clinical scenarios will be reviewed, such as management of complex cases and use of specialized equipment in the standalone setting as well as anticipation, avoidance and management of complications. The information provided will be summarized as a framework for policy considerations for setting up stand-alone angioplasty programmes across the country.

The landscape of antiplatelet therapy is rapidly changing, with recent landmark trials providing critical efficacy and safety information on new and existing therapies for Acute Coronary Syndromes (ACS). Therefore, there is a clear need for contemporary perspectives by experts in this area, so healthcare practitioners delivering ACS care to Canadian patients can better interpret the role of all emerging agents, in order to optimize long-term patient outcomes. This symposium will aim to gain a better understanding of the mechanisms and profiles of antiplatelet therapies, in order to provide a foundation for exploring the future standard of care for treating ACS.

First approved in the 1950s, warfarin remains to this day the most widely prescribed anticoagulant in North America. Now, almost sixty years since its introduction, warfarin might soon be eclipsed by important and potentially practice-changing advances in medical science, offering physicians a new generation of agents that have already, and may continue, to prove safer and more effective for preventing stroke. This new era of oral anticoagulation therapy is arriving with much promise. Many questions are yet to be answered. How do they compare and contrast the totality of data from the landmark clinical trials-ROCKET-AF, RE-LY, ARISTOTLE, AVERROES and translate it to clinical practice? What role, if any, will warfarin still have for patients with good INR control? The accredited symposium Focus on SPAF: Ask the Expert, will seek to answer these questions and more. An international panel of experts will convene to present the results from these landmark trials and analyze this data.  Canadian leaders treating stroke and atrial fibrillation will join them to determine what the implications will be for clinical practice in our country. And in the spirit of open dialogue, health care professionals from coast-to coast-to coast will join the symposium using telepresence technology to engage these world-renowned panelists and their Canadian colleagues with questions.

This session was co-developed with the Canadian Cardiovascular Society, St. Paul's Hospital Foundation and
Edwards Life Sciences, and was planned to ensure the evidence presented is valid, objective and balanced.

The program is a live demonstration of two cases transmitted from St. Paul's Hospital in Vancouver. The first case will describe the planning and realization of a procedure aiming at recanalization of a chronic total coronary occlusion (CTO). Novel techniques have greatly enhanced procedural success for this complex coronary lesion. The second case is a transcatheter aortic valve implantation (TAVI). TAVI has emerged as an interesting alternative to surgical aortic valve replacement in high risk patients with severe aortic stenosis.

The priority will be to engage the audience in the discussions. Participants will be able to view the cases and ask questions to the operating physicians and panel members during the procedures.

Learning Objectives:
At the conclusion of this workshop, participants will be able to:

• discuss technological and pharmaceutical developments in the field of interventional cardiology;
• develop new clinical approaches to coronary and valvular diseases using imaging and less invasive procedures
• apply guidelines and understand new therapeutic strategies applicable in their clinical practice; and
• assess the patient eligibility criteria for coronary and valvular procedures, potential complications, and patient management prior to and after the procedure.
